How do I request leave due to COVID-19?
- Step 1 – Click on the following link to see if you qualify for paid leave for COVID-19: Covid-19 Leave Summary
- Step 2 – Notify your direct supervisor that you need to take a leave due to COVID-19. Please do not add your leave in AESOP.
It will be added for you once the leave is approved.
- Step 3 – Request a COVID-19 Leave Form from Carrie Meyer at [email protected].

- Step 4 – E-mail your completed leave request form along with all documentation necessary to [email protected]
- Step 5 – You will receive an e-mail from HR when your leave has been approved and your approved COVID-19 days will be added
to AESOP.
Note: Intermittent leave is only available if an employee is caring for his/her child (under the age of 18) whose school or
place of child care has been closed, or the child care provider is unavailable due to COVID-19 precautions.
